Welcome to Social Care Solutions
Social Care Solutions provides high quality Supported Living schemes and Outreach services for adults with Learning Disabilities, complex needs and associated challenging behaviours. We also provide a wide range of day care services, including our own high quality Snoezelen facilities.
Social Care Solutions has always been a passionate advocate of responsible empowerment. We believe this is how our Service Users will develop as individuals and achieve self-determination. Our aim is to be a guiding hand; a discreet safety net within which we encourage and support them to lead ever-more satisfying and fulfilling lives.
We are now driving forward the change towards Self Directed Support. Having adopted the In Control Jigsaw Framework to focus awareness and implement the Self Directed Support agenda, we are enabling individuals to manage and direct their own support on a daily basis. We encourage service users to think about the outcomes they would like and to use the funds/hours available to them to achieve those outcomes. This means service users are fully involved in deciding how their support is going to be used in the weeks ahead.
Social Care Solutions fully embraces the Department of Health’s Valuing People Now strategy. We uphold our Service Users’ right to each be treated as an individual and we respect their individual rights to privacy, self-determination, dignity and to a safe and secure living environment. We also ensure that our Service Users are able to pursue all legitimate lifestyle choices and ensure each individual’s Person-Centred Care Plan fully reflects their own personal wishes, needs and beliefs.
Our Philosophy of Care
We aim to encourage individuals to develop their independent living skills as well as supporting the development of social and interpersonal skills. We are committed to providing a user focused service in partnership with the individual who has a disability. This involves four main principles, Respect, Trust, Development and Equality.
Respect
We respect the individual and value their right to make their own decisions. To enable this we assist Service Users in designing their own personal development plans. The PDP incorporates a "likes and dislikes" list, Daily Activities Programmes, Targeted Behaviour Guidelines and long and short term goals. The PDP is unique to the individual and is reviewed on a regular basis. Our aims is to make each Client as individual as they want to be.
Trust
Each Service User is assigned a key worker whose role it is to closely work with the person to ease any anxieties and to learn about their wishes. The key worker establishes a trusting relationship with the Service User based on honest and mutual respect.
Development
Working together staff and Service Users learn and develop from each other. The knowledge that each acquires from the other effectively helps build understanding, mutual respect and partnership.
Equality
All Service Users are regarded as individuals who are valued equally.
